|
Double checking can be divided into two mutually complementary practices : detection of prescription error before dispensing and detection of dispensing error after preparation of drugs. Data about double checked prescriptions, that have been accumulated for 2 years, were analysed. Results show that 112 events(0.04％) out of total 291, 290 prescriptions were dispensing error : the most frequent dispensing error occurred in the medicine for external application and internal solutions (23.2％), followed by misunderstanding of prescription (21.4％) and dividing error (16.1％). According to this study, application of multistep checking and continuous education for pharmacists are desirable.
